This application is to be completed by all volunteers. It is used to help the Ministry provide a safe and nurturing environment for its families and children, and to maintain a current database of worker addresses for effective communication.  The following questions are very personal and for a variety of reasons may be difficult and painful to answer.  If you are struggling with any of the following issues, our desire is to support and encourage you in your journey of healing.  Your responses do not automatically exclude you for being considered for a position, but will help us determine your readiness for ministry at this time.   
 
All information contained in this application will remain confidential except as restricted by Arizona State Law A.R.S. 36-2281 which requires we report neglect, physical, or sexual abuse that you might choose to reveal in the process of completing this application.  If you do reveal that you are currently or have sexually abused children under the age of eighteen we will walk through the process of reporting the abuse to the authorities, but will also engage in conversation regarding our desire to connect you with a church who is willing and desirous of being part of your process of accountability, restitution, repentance, and healing.
 
As part of the application process and Neighborhood Ministries, you will be required to sign a waiver allowing us to obtain Federal and State background clearance.

We work with a very vulnerable population and want to provide a space as safe as possible for them as well as for our volunteers.  If you are struggling with this question, please make a note and we'll connect with you personally.
